  1. You are at least 18 years old.
  2. Your contribution is made from your own funds, and funds are not being provided to you by another person or entity for the purpose of making this contribution.
  3. You are a U.S. citizen or lawfully admitted permanent resident (i.e., green card holder).
  4. You *must* include your Full Name, Address, Occupation, Employer, and Employer’s Address along with the check.
